Fat transfer or lipofilling is usually used to restore volume. However, over the past decades fat grafting has undergone a renaissance in aesthetic and reconstructive surgery. It was noted that adipose tissue encompasses mesenchymal stem cells. Although the regenerative process is still not fully understood, the positive regenerative effect on the surrounding tissue is well recognized. The overlying skin quality, lines and scars improve. Enriched fat transfer takes this a step further by supplementing a patients own regenerative cells to create a natural filler.
During the aging process our face will gradually loose volume, ie fat. This is the reason why patients who have a full face when they are young, often look younger than their peers when they get older. Basically, because they had more facial fat to begin with. The fatty volume loss is influenced not only by genetics, but also by environmental factors like smoking or hormonal changes. Women often notice in their late 30’s, early 40’s, a volume loss in the lower parts of their cheeks. The fatty tissue is stored in distinct deep and superficial compartments in our face. Over time the fatty tissue decreases and the overlying skin has less volume support. Consequently the skin elasticity decreases, resulting in facial lines, sagging and a hollow appearance. The volume loss does not only affect the cheeks, often areas just below the eyebrow, temples, lips, and lateral orbital wall area are affected as well.
Fat grafting is a procedure in aesthetic plastic surgery that helps patients to smooth and to restore volume to their face leading to a more youthful, healthier appearance. Micro and nano fat grafting techniques have advanced facial rejuvenative procedures. Many patients seeking a facelift procedure or a blepharoplasty are benefitting from an additional fat transfer.
Micro and nano fat grafting techniques work in a similar way to injectable fillers, but using your own tissue, there is no risk of allergic reaction. The fat for the transfer is harvested by liposuction. The gained adipose tissue will then be processed, purified, and injected into the desired area.
Due to its rich blood supply, the face is an ideal recipient for lipofilling. Lipofilling procedures can used as a natural filler, or can be combined with other surgical procedures.

Patients having facial wrinkles, like crows feet, smile lines, volume loss of their cheeks, temples, upper eyelids, lips, visible infraorbital rim, dark circles of the lower eyelid are usually good candidates for fat transfer procedures. Since only a small amount of fat needs to be harvested, facial fat transfer is an excellent procedure even in lean patients.
If you are a healthy patient seeking improvement, you are most likely an excellent candidate for a lipofillng procedure.
Fat is harvested from an area of your body using liposuction. The fat will then be purified and carefully transferred. The harvesting process can be either done using a local anesthetic or in combination with another surgical procedure, when you are already under general anesthesia. Unfortunately, your harvested fat cannot be frozen or stored, therefore, a fat transfer may have to be repeated. The cells would not survive the freezing and thawing process. Like in other human tissues, storage at an official tissue bank may become available in the future. After the lipofilling expect some bruising and swelling of the donor and recipient site which will gradually subside. Dr Knoll and her staff will do their best to help you achieving the most beautiful and natural-looking results, as well as making the entire surgical fat transfer experience as comfortable as possible for you.
