Chin ptosis, unflatteringly also known as “witches chin”relates only to the overlying soft tissue of the chin. Patients show a soft tissue fullness overlying the bony chin that exceeds vertical and is usually accentuated by the skin crease underneath the chin. The vertical excess of the soft tissue of the chin, appears to be sagging or being ptotic. Existing chin ptosis is often exaggerated when smiling. The chin ptosis and its related vertical excess can not only alter facial proportions of the lower third of the face, the anterior part of a well defined jawline is offset.
Chin ptosis surgery, or the correction of the “ witches chin ” is a relatively straight forward procedure which can be performed under local anesthesia in most patients. The incision is placed inconspicuous underneath the chin. The usually surgery associated restrictions, swelling, bruising and pain, are kept to a minimum.
However, there are several important considerations related to outcome and scar placement. The amount of soft tissue excess, the position of the skin crease and the related bony width, the underlying bony shape of the chin, and potential previous filler injections or fat grafting procedures.These vary usually from patient to patient and should be discussed during the initial consultation.
Patients who are requiring any additional bone work on their chin, alternating their shape or projection will require some form of general anesthesia. Chin surgery, either augmentation or reduction, or just a simple chin contouring procedure are usually performed as a one day, out patient, procedure.
Understanding a patients concerns and a thorough pre-operative assessment by Dr Knoll, allows patients to learn about their options. Provided information about previous filler injections and / or fat grafting procedures is of great help. In some instances X-rays and/or a facial CT-scan (bone windows) are requested to assist in preoperative planning.
