The “Internal Bra” technique is a surgical approach to support the lower portion of the breast when needed. It is often compared to an internal “push-up bra”. Some imagine it as an internal “hammock”. The goal is to create an internal support to prevent bottoming out and to maintain position over time. The breast shape and the aesthetic appearance can be enhanced attaining fullness and cleavage. The “Internal Bra” technique is not needed in all types of breast surgery. But the loss of elasticity and poor natural support of a patients own tissue, often benefit from additional internal support placed during surgery. This is especially true, when a patients breast tissue is very heavy or when a larger, heavier implant is or was placed. Sometimes additional support is needed, when a patient had to undergo corrective procedures involving a total or en-bloc capsulectomy after a breast augmentation. With the removal of the breast implant capsule, the space enlarges, often requiring additional support, when a new breast implant is placed.
There are different surgical techniques to create the so-called “internal bra”, giving support and shape to the breast. The surgical approach of an “internal bra” can use a patients own tissue, certain scaffolds, or mesh-like material that will either integrate or resorb over time.
Undergoing breast surgery or a revisional procedure can be anything, from straight forward to a very complex procedure. In many breast surgeries the “internal bra” concept is not needed. However, there are some initial breast surgeries benefitting from this concept. The complexity certainly increases for example, when a breast implant exchange is desired with a capsular contracture being present. A patients medical and surgical history plays an important role when making a consensual decision encompassing a patients desired outcome, potential risks and realistic expectations.
Which kind of internal bra technique can be applied is dependent on a patients individual situation, their anatomy and aimed aesthetic outcome. There is no one size fits all, but certainly a better or worse option for a patients individual problem.
